Making a claim

If you have been injured in a motor vehicle accident you may be able to make a CTP claim with Allianz if the vehicle at fault was insured by Allianz. This includes accidents where you were the driver at fault and insured by an Allianz CTP policy.

If you submit a claim within 28 days, you may be entitled to receive benefits starting from the date of the accident. If you submit later than 28 days from the accident, some of your entitlements may only commence from the date the claim is made. At the latest you must lodge a claim within 3 months of the date of accident.

If you do not know whether Allianz is the CTP insurer of the at fault vehicle, please contact us on the number above and we will advise you.

If the at-fault driver is not insured with Allianz for CTP, you can contact SIRA to determine who the at-fault driver is insured with here.

What you need to do before making a claim

  1. Obtain the registration details of the vehicles involved in the accident.
  2. Report the accident to the police within 28 days from the date of the accident (if the police did not attend the accident scene.) You can report the accident to the NSW Police by contacting the Police Assistance Line on 131 444, 24 hours a day, 7 days a week.
  3. Download and complete a claim form from our forms section below, or from the State Insurance Regulatory Authority (SIRA) website using the link below. If you would like help completing the claim form, please call our claims team on 1300 131 319 who will be happy to assist.

How to make a claim

If either you are the at fault driver and insured with Allianz or the at fault driver is insured with Allianz and wish to make a claim, please complete and send to us the claim form (links provided below).

  • You need to send the completed form to:

  • You can also email it to us at CTP_NSW_rego@allianz.com.au, but please also send the original copy in the mail.

    The following is a guide to assist you on how to complete the form. If you require assistance or have any questions, please contact us and we will be happy to assist you.

    1. Print a copy of the Application for personal injury benefits. If you are unable to print this form, please contact us and we will send the form to you. Alternatively, we will fill it out for you from the information you provide over the phone, and send a completed form for you to sign and return.
    2. The form should be sent to us as soon possible as the law sets time limits for making a claim. You must provide the form to us within 3 months of the date of accident, or within 28 days to maximise your entitlement to benefits. If you submit a claim within 28 days, you may be entitled to receive benefits starting from the date of the accident. If you submit later than 28 days from the accident, your treatment and care benefits may still commence from the date of the accident but benefits for income support may not commence until the date the claim is made.
    3. You must include a medical certificate from your GP detailing your fitness for work. A copy of the certificate of fitness is available from SIRA's website here.
    4. If you are making a claim for funeral expenses then you should complete an Application for funeral expenses form.
    5. If you are claiming for losses or expenses incurred as a relative or a dependant of a person who died in a motor vehicle accident, then you should complete an Application for compensation to relatives form.
    6. Once the claim form is completed, you will need to sign it. If you are under 18 years of age, your parent or legal guardian over the age of 18 years should sign it on your behalf.
    7. If you wish to make a claim for common law damages, then complete an Application for damages under common law form. You can only claim for damages if you meet the criteria set out at the top of the form.

    Yes, the legislation imposes time limits for making a claim. For a personal injury statutory benefits claim, you must submit a claim no later than 3 months from the date of the accident. If the claim is made outside of the 3 months, we may require additional information to consider accepting your claim.

    Please contact us if you require further information about the time limit to make your claim, or about time limits for other types of claims.

    We will contact you within 3 business days after we have received your claim form. We will send you a letter advising you of your claim reference number and the name and contact details of the person who will be responsible for supporting your recovery journey. We will also keep you up to date on the progress of your claim and encourage you to contact us if you have any questions.

    We may need to contact you to obtain an update on the progress of any treatment and we may also need to obtain further documents from you to assess your claim. We will discuss with you the need for any of this additional information.

    If you have been injured in a motor vehicle accident and wish to make a claim for your injuries, see below.

    You may be able to make a CTP claim with Allianz if:

    1. You have suffered an injury as a result of a motor vehicle accident that was not your fault; and
    2. The vehicle at fault in the accident was insured by Allianz CTP.

    There are strict timeframes in relation to making a claim if you were injured before 1 December 2017. Please contact us if you require more information.


    What do I need to do before I can make a claim?
    1. Ensure the accident has been reported to the Police (if they did not attend the accident.) You can report the accident to the NSW Police by contacting the Police Assistance Line on 131 444, 24 hours a day, 7 days a week. Tell the police you were injured in the accident and get the police event number of the accident.

    What happens next after I have sent my claim form to Allianz?
    • We will contact you within 3 business days after we have received your claim form. We will send you a letter advising you of your claim reference number and the name and contact details of the person who will be responsible for supporting your recovery journey. We will also keep you up to date on the progress of your claim and encourage you to contact us if you have any questions.
    • If your claim was lodged more than 6 months after the date of accident, you may be asked to provide an explanation for the claim being delayed.
    • We will make enquiries into the circumstances of the accident and determine whether the Allianz insured driver is at fault. We are required to determine liability as soon as possible.
    • We will let you know your entitlements and your claim will be assessed accordingly.
