How to apply
Applying is easy. It usually takes about 10 minutes (but can take longer depending on your medical history).

To complete your application you will need the following:
- Your partner present, if you and your partner are both applying
- Details of how much you drink and/or smoke
- Your height and weight
- Your medical history including illness, injuries and any medication
- Your family's medical history if known
- Your Doctor's name and contact details
- Credit card and/or bank account details
Retrieving a saved application
At any time during your application you can save the information and retrieve and complete later. Applications are available to be retrieved and completed for up to 30 days.
